Community Service Forms

• Community Service Handbook. This is a comprehensive guide for anyone planning service through ASU. It will give you pointers on planning, budgeting, evaluating and reflecting.
• Liability Release Form. This form is a necessary protection for anyone doing service or traveling in the name of Arizona State University . More details on this form are available online [Web site forthcoming]
• Photo Permission Release Form. If you want to take pictures, be sure you have a signed release form before using the photos in any way (Web sites, publications, etc).
• Sample Event Sign-In sheet (under construction). It's always a good idea to have a sign-in sheet at your event. This aids in assessment and follow-up once the service is complete.
• Sample Service Reflection Outline (under construction). Service is a big deal, so be sure to wrap up with reflection time. This outline will provide some possible questions and follow-up activities for your group.
